Build Your Registration Form
Last updated
The registration form is what families fill out to sign up. Lazarus includes the essential contact and attendee fields automatically, and you can add your own questions on top — allergies, T-shirt sizes, a photo release, whatever this event needs. This guide covers the default fields, adding questions, and deciding whether a question is asked once per registration or for each attendee.
Where to Build Your Form
Open the event from the Events page. Below the roster you'll find the Custom questions panel, which lists your questions in the order families see them. Click Edit questions to open the form builder.
Questions are set up per event, so each event can ask exactly what it needs.
Default Fields
Every registration automatically collects:
- Once per registration: Email address, First name, Last name, and Phone (optional)
- For each attendee: First name and Last name
These are always part of the form, so there's no need to add your own name or email questions. Families your school already knows get their details filled in for them — see Invite Families & How Registration Works.
Adding a Question
In the form builder, choose where the question Applies to, pick a Question type, and enter the Field name or question families will see. You can add Helptext for extra instructions and switch on Make this required for anything families must answer. Then click Add question.
The available question types are the same as on enrollment applications:
- Text — a single line of free text.
- Email — a text field validated as an email address.
- Phone — a text field formatted for phone numbers.
- Date — a date picker.
- Number — numeric input only.
- Dropdown — a list families choose one option from. Enter the options separated by commas.
- Multiple choice — a set of options presented for selection. Enter the options separated by commas.
- Yes or No — a simple yes/no toggle, good for consent and permission questions.
- Textarea — a larger box for longer answers.
- Upload — lets families attach a file, such as a signed permission slip.
Registration vs. Attendee Questions
Each question has a scope that controls how often it's asked:
- Registration — asked once, of the person registering. Use this for anything that applies to the whole group, like an emergency contact for the day or how they heard about the event.
- Attendee — asked for each attendee. Use this for anything specific to a person, like dietary needs, T-shirt size, or a child's grade.
For a family registering three people, an attendee question appears three times — once under each name — while a registration question appears once. To change a question's scope later, open its ⋯ menu.
Editing and Reordering
You can come back to the form builder at any time. Drag questions to reorder them, use a question's ⋯ menu to edit or delete it, and close the builder when you're done. The Custom questions panel on the event page updates to match.
Important: Before you can publish an event, Lazarus asks you to confirm that you won't use its questions to collect sensitive personal data from minors — Social Security numbers, government IDs, financial account details, or full medical histories. If you need that kind of information, collect it through a more secure channel.
Tip: Keep event forms short. A family signing up for a field trip at 9pm on their phone will finish a three-question form, and may abandon a twelve-question one. Ask what you need for the event itself, and nothing more.
With your form built, the next step is publishing your event and sharing the link.
